Company has entered into contracts to provide certain services to Medicare Advantage organizations (“Health Plans”). Health Plans have entered into contracts (each, a “MA Contract”) with the Centers for Medicare and Medicaid Services (“CMS”). This Exhibit is incorporated by reference into and made part of the Agreement with respect to services rendered to Patients enrolled in the Medicare Advantage program (“MA Members”). In the event of a conflict or inconsistency with any term or condition in the Agreement relating to services rendered to MA Members, this Exhibit shall control.
