THIS AGREEMENT (“Agreement”) is entered into as of the day of , 2016 (the “Effective Date”) by and between Trinity Health ACO, Inc., a Delaware nonprofit corporation (“Company”), and the entity or individual identified on the signature page hereof (“Preferred Provider”), on behalf of itself and, if applicable, its Providers (as hereinafter defined). Company and Preferred Provider are the only parties to this Agreement.
