supplement and are made part of each agreement held by Medica Holding Company, LLC, or Medica Service Company, LLC, or one of its Affiliates (individually and collectively, “Medica”) and each Agent or Agency, a Corporate Agency Partner, Field Marketing Organization or Medica Agency Partner (referred to collectively herein as “Agent or Agency”) (the “Agreement”). These Requirements are effective as of the date of the Agreement. In the event of a conflict between these Requirements and the Agreement, these Requirements will govern with respect to services related to Medica’s participation in Medicare programs. This Addendum, effective upon issuance, shall replace any previous version of the Requirements posted on the Broker Portal.
