(“Addendum”) is entered into by Hawaii Dental Service (“HDS”) and participating provider (“Provider”). This Addendum supplements the Member Dentist Agreement (“Agreement”) entered into by HDS and Provider. HDS and Provider may be referred to individually as a “Party” and collectively as the “Parties.”
