This MPA is effective upon final signature (Florida Contract Number 42000000-18-ACS) and commences October 23, 2018 or earlier if agreed upon between the State of Florida and Henry Schein Medical a division of Henry Schein Inc. (MMS18016) or by any party upon 30 days’ written notice to the other parties to this MPA listed below, and expires on June 25, 2020, unless the applicable annual extensions are executed.
