This agreement provides membership in the ambulance subscription service known as the AFMA Ambulance Membership Program. The annual membership fee per household (‘household’ defined as legal spouse or domestic partner including children under the age of 26, living at the address) provides emergency ground ambulance service and medically necessary non-emer-gency ground ambulance service authorized by a physician within the approved AFMA service area, to the nearest appropriate medical facility, for the subscriber and household members for one year, effective on the first day of the month following receipt of payment. Membership also provides physician-authorized, medically necessary ambulance service from a point within the AFMA approved area to any other hospital within the State of Arizona. The annual fee for this service is currently be $75.00 per household.

We are pleased that (“you” or “your”) are joining the Membership Program at Greektown Neighborhood Partnership, a Michigan non-profit corporation (“GNP”, “we”, “our” or “us”). The Membership Program allows GNP to provide services to the Greektown neighborhood and to its Members and strengthens the commitment of our community to the long-term economic and cultural vibrancy of Greektown. Each party’s respective rights and obligations are outlined below. This Agreement shall be subject to and incorporates the extended GNP “Membership Program Agreement”, which can be found at www.greektowndetroit.org, as updated from time-to-time. By entering into this Agreement, Members are thereby subject to all terms and conditions outlined in the GNP Membership Program Agreement. This Agreement may not be amended or modified except by an instrument in writing signed by authorized persons of each party. This Agreement shall be accepted, effective and binding, for all purposes, when the parties have signed and transmitted to each other, by facsimile or e-mail, copies of an executed version of this Agreement.

The research will be directed by Rosalie Ocker of the University who will in her capacity on the project as the Executive Director of the Center for Enterprise Architecture and not as a consultant to the member.
