Disasters can happen at any time. Demands for community support services may overwhelm our existing capabilities. Past disasters have shown that coordination of like services, within our communities, makes us stronger, is cost effective and maximizes the use of scarce resources. Further, disaster coordination is complex and requires communication and cooperation among all non-profits, community-based organizations (CBOs), private sector, and all levels of government.

This is to confirm that _________________________ (name of network partner/organization), located at _____________________________________________________________________ (address of network partner/ organization), has agreed to be a provider in the network of Prime Care Coordination LLC. As a member of the network, __________________________ (name of network partner/organization) will receive referrals and provide services (e.g., behavioral health, physical health, social and community supports, specialty children’s services, developmental disability services, and other long term supports and services) and collaborate and support the work of the CCO/HH care managers who will develop Life Plans for individuals with intellectual and/or developmental disabilities who will be enrolled in and receive Health Home care management through Prime Care Coordination LLC.
