This Network Provider Pharmacist Contract is between the Office of Management and Enterprise Services Employees Group Insurance Division (EGID), (hereinafter, EGID) and the Pharmacist who agrees to the terms of this agreement by signing the appropriate Signature Page. The designation of “Pharmacist” in this agreement refers to the person who signs this agreement as the Pharmacist.
