This contract is between Pinnacle Health & Rehab, 26 Pleasant Street, Canton, ME (the “Facility)” andNursing Facility Admission Contract • April 4th, 2018
(the “Resident”). It will be signed by the Resident or someone authorized to sign for the Resident (the Resident’s Agent*). It describes the Resident’s financial obligations, as well as other responsibilities and rights. It also describes the rights and obligations that apply to the Facility in the course of providing care to the Resident.
