In consideration of being allowed to utilize the facilities of the agenda in conjunction with the Ohio University Medical Assisting Technology Program, I hereby agree to assume full responsibility for my own safety and indemnify, save and hold harmless, the agency, their employees and agents, acting officially or otherwise, from and against any and all employees and agents, acting officially or otherwise, from and against any and all liability, claims, demands, actions, debts, and attorney fees arising out of claimed on account of, injuries to, or death of any and all persons whatsoever, in any manner caused or contributed to by me while in, upon, or about the agency facilities, in connection with the aforesaid program, and to indemnify the agency from an account of damages of any kind which the agency may suffer as a result of my action in or about the facilities and premises of the agency.
