(“Agreement”) is entered into as of the last date of signature below ( “Effective Date”), by and between Campus Clinic, LLC., a Delaware limited liability company, with its principal office located at 1401 21st Street STE R Sacramento, CA 95811 (the Lead Agency and referred to as “Campus Clinic”), and Yuba City Unified School District, a California school district, with its principal office located at 750 Palora Avenue, Yuba City, CA 95991 (“District”). Campus Clinic and District are referred to herein individually as “Party” and collectively as “Parties.”
