This Online ServiceLink Order Form is part of the CLEANTELLIGENT SUBSCRIPTION AGREEMENT between CleanBrain Software, Inc. (hereinafter “CleanBrain”) and the company or individual defined below (hereinafter “Client”) and all exhibits and addendums thereto including, but not limited to, this Online ServiceLink Order Form, as determined by CleanBrain (hereinafter “Agreement”). The terms and conditions of the Agreement along with its definitions including, but not limited to, the definition of CleanBrain, are referenced in and/or attached to this Online ServiceLink Order Form. By signing below, Client agrees to the Agreement and its exhibits and addendums, including this Online ServiceLink Order Form.
