This Business Associate Agreement (this “BAA”) is entered into by and between Ortho2, LLC (“Business Associate”) and the existing or potential Ortho2 customer named below (“Customer”). Each is a “Party” and collectively “Parties”, who entered into, or may enter into, the Service Agreement (as defined below) with Business Associate. This BAA, in addition to standing on its own, hereby amends and is incorporated into the Service Agreement between Customer and Business Associate. To the extent that the provisions of this BAA conflict with those of the Service Agreement, the provisions of this BAA shall control.
