THE HEALTH AND HUMAN SERVICES COMMISSION (“System Agency”), a pass-through entity, and El Paso Community MHMR dba Emergence Health Network (“Grantee”) who are collectively referred to as the “Parties” to that certain Outreach, Screening, Assessment and Referral (SA/OSR) services contract, effective September 1, 2020, and denominated HHSC Contract No. HHS000785200008 (the “Contract”), as amended, now desire to further amend the Contract.
