The Health and Human Services Commission (“System Agency”), a pass-through entity, and Andrews Center (“Grantee”) (each a “Party” and collectively the “Parties”) to that certain Outreach, Screening, Assessment, Referral (SA/OSR) services contract, effective September 1, 2020, and denominated HHS Contract No. HHS000782500012 (the “Contract”), now want to amend the Contract.
