Outreach Serve is a volunteer-based ministry of Church of the Highlands, Inc., which shows God’s love in practical ways through serving the needs of communities and residents, including times of natural disaster or crisis (the ministry’s operations are collectively referred to throughout this Agreement simply as “Outreach Serve”). I give permission for my child or legal ward (insert child’s name here) (hereinafter referred to in this Agreement as “my child”) to serve as a volunteer with Church of the Highlands’ Outreach Serve. In exchange for my child being allowed to participate in Outreach Serve and for other good and valuable consideration provided by Church of the Highlands, the receipt and sufficiency of which I acknowledge, I, on behalf of myself and my child, freely, voluntarily, and without duress agree to the following terms and conditions:
