We (or I) hereby request that you accept this application for the 2016 CHAMPIONSHIP BASKETBALL CAMP, INC. during the dates marked. Upon your acceptance of this applica- tion, we (or I), whether one or more, hereby release Championship Basketball Camp, Inc., Johnny Tauer, University of St. Thomas, Macalester College, Nativity Grade School, Colin Powell Center, and all their respective employees and coaches from all claims on account of any injuries which may be sustained by our (or my) son or daughter while attending CHAMPIONSHIP BASKETBALL CAMP. We (or I) agree to indemnify Championship Basketball, Inc., Johnny Tauer, University of St. Thomas, Macalester College, Nativity Grade School, and Colin Powell Center and all its employees for any claim which may hereby be presented as a result of such injuries. We (or I) also authorize the coaching staff to call medical person- nel to treat our (or my) son or daughter in the event of illness or injury.
