Please note: By completing and signing this form you are entering into a contractual agreement with the provider for the whole term. This means that you are not able to change your child’s funded hours after Headcount Day on 7 May and you cannot transfer your child’s funding. Therefore, if you decide to move your child to another provider you may become liable to pay childcare fees to the new provider. Please make sure you read the Important Information section before signing this form.
