This contract sets out the expectations of both parties as to the care of your child, activities provided and business arrangements. If agreeable, would you please sign both the contract and the slip at the bottom of the page and return to the Playgroup leader/Manager who will then also sign both sections. The signed slip will be retained by you as the child’s parent/guardian and the signed contract retained by the setting.
