The undersigned participant (the “Participant”) submits this Participant Application and Agreement (the “Application”) to Catholic Fraternity Fund, LLC (“Catholic Fraternity Fund”) to participate in the Income Emphasis Fund, Balanced Fund, or Equity Emphasis Fund (the “Funds”) under the administration of the Catholic Community Foundation. This Application may be accepted or rejected by Catholic Fraternity Fund in its sole and absolute discretion.
