In consideration for my or my child’s participation in___________________________________ (“Activity”) hosted by _______________________________, taking place on the University of Florida campus or in a University of Florida facility, on or about ___________________, I hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE the University of Florida Board of Trustees and their respective employees, agents, representatives, officers, trustees, members and volunteers (“RELEASEES”) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, my child, or to any property belonging to me or my child, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES or otherwise, while participating in Activity or while in, on or upon the premises where Activity is being conducted.
