The ACO has entered into an agreement (“MSSP Agreement”), with the Centers for Medicare & Medicaid Services (“CMS”) to participate in the Medicare Shared Savings Program (“MSSP”) to provide services to Medicare fee-for-service beneficiaries (“MSSP Enrollees”) assigned to the ACO. This Agreement allows Participating Physicians and other licensed health care providers employed by, or contracted with, the Practice and who bill for items and services furnished to Medicare fee-for-service beneficiaries under the Medicare billing number assigned to the tax identification number (“TIN”) of the Practice (each, a “Provider”) to participate in the ACO’s network of providers (“Network”) with respect to the MSSP Agreement. THIS IS AN AMENDMENT (“Amendment”) to the Meridian Accountable Care Organization LLC Participating Practice Agreement (“Agreement”) by and between Meridian Accountable Care Organization LLC (“Meridian ACO”), and the practice identified on the signature page hereof (the “Practice”) and physician(s) (“Physician” or “Participating Physician”) identified on Exhibit A. Meridian ACO, the Practice and the Participating Physician are the “Parties”, and each is a “Party”. This Amendment is effective as of the date it is signed by Meridian ACO.
