The purpose of this Agreement is to enable parents and children to give informed consent for that player to participate in the lacrosse program (this program includes conditioning training, lacrosse practices, clinics, games, tournaments and are referred to collectively as the “program”) for Central Dauphin Rams Youth Lacrosse (“CDRYL”) and to confirm the agreement of the player and the parents regarding assumption of risks, waiver and release of liability, and indemnification, as an express condition of each player’s participation in CDRYL’s lacrosse programs.
