This Pay for Performance Funds Agreement (this “Agreement”) is made effective as of March 1, 2018 (the “Effective Date”) by and between Adirondack Health Institute, Inc., a New York not-for-profit corporation with its principal location at 101 Ridge St, Glens Falls, NY 12801 (“AHI”) and , with its principal office at (the “Participant”). AHI and the Participant are each referred to herein as a “Party” and collectively as the “Parties.”
