This PCMH Care Coordination Contract (the “Agreement”) is hereby made and entered into between Community Care of North Carolina, Inc., a North Carolina nonprofit corporation whose principal office is located in the City of Raleigh, County of Wake, State of North Carolina, (“CCNC Inc”) with a satellite office in Little Rock, Arkansas, and , an Arkansas primary care practice whose principal office is located in the City of , County of , State of Arkansas, (“Practice”) (each a “Party” and collectively the “Parties”) as of the effective date provided for herein below, for the purpose of enabling CCNC Inc to support practices participating in the Patient Centered Medical Home effort under the Health Care Payment Improvement Initiative (the “Plan”) in the care coordination of their patient population.
