We, the undersigned faculty member and graduate student, agree that the faculty member will be the major professor for the student’s Ph.D. dissertation committee. We understand that this form must be submitted to the Department before the end of the student’s second academic year in residence. We understand that both parties (student and major professor) retain the right to withdraw from this agreement.
