This Physician Designs Services Agreement (the “Agreement”) by and between PHYSICIAN DESIGNS LLC (“Physician Designs” or “We” or “Us”), and you, (“Customer” or “You” or “Your”) is entered into as of the effective date below set forth next to Customer’s signature on the final page of this Agreement. In consideration of the mutual promises and representations to be performed as set forth below, and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, Physician Designs and You agree as follows:
