THIS AGREEMENT is made by and between the WPPA, INC., a corporation organized under the laws of the state of Kansas (hereinafter referred to as "WPPA"), and the Participating Provider and the Group who mutually agree to participate in the WPPA's Provider Program by executing a Declaration of Agreement incorporating by reference all of the terms and conditions set forth in this Agreement.
