In consideration of being allowed to enter the swings N Things play areas, the undersigned, on his or her own behalf of the minor(s) identified below acknowledges, appreciates, and agrees the following:

By signing below, the undersigned “Play Pass Holder” agrees to a 12-month Play Pass at Carter Plantation, starting on the date of execution. Play Pass Holder may cancel this 12-month Play Pass at Carter Plantation by written notice provided to Carter Plantation within 30 days of its execution of this Agreement.
