The Health and Human Services Commission (“HHSC” or “System Agency”) and Health Center of Southeast Texas (“Grantee”), who are collectively referred to herein as the “Parties” to that certain Primary Health Care Grant Contract that was effective September 1, 2021, and denominated as HHSC Contract No. HHS000697900039 (the “Contract”), as amended, now desire to further amend the Contract.
