This document is the First Amendment to that certain Professional Services Agreement dated September 19, 2016 and that certain Exhibit A thereto titled Statement of Work (collectively, the “Agreement”) between and among Proactive MSO, LLC as assignee of Wellness for Life Medical LLC (“Proactive” or “Service Provider”) and Tippecanoe County Government (“TCG” or “Client”). This Amendment is effective as of the Amendment Effective Date set forth on the signature page hereto (the “Amendment Effective Date”).
