ContractProgram Co-Sponsorship Agreement • February 7th, 2013
The Arthritis Foundation (“AF”), Florida Chapter (“AF Local”), has agreed to co-sponsor the following AF programs: Arthritis Foundation Exercise Program, Arthritis Foundation Aquatic Program at the City of Hallandale Beach (“Co- sponsoring Agency” or “Agency”), with its facility location at Peter Bluesten Park in Hallandale Beach, FL (and locations listed on Addendum A). This co-sponsorship is contingent upon the following conditions, to which the parties agree by the signatures of their representatives below:
