THIS AGREEMENT is made between WINTER4KIDS, its representatives, agents, officers, directors, servants and employees (collectively referred to as “WINTER4KIDS”), and the undersigned (referred to as “I”). WINTER4KIDS AND I agree as follows:

By submitting this application, I certify that I am the parent/guardian of the individuals listed above and give permission for him/her to participate in the programs selected. This agreement is made upon the condition that I assume all risks and hazards incidental to the individual’s participation and do hereby waive, release, absolve, indemnify, and hold harmless the Borough of Red Bank, the Department of Parks and Recreation, and its agents and employees for any claim arising out of injury to the individual listed on this form. I acknowledge that they are expected to follow the Code of Conduct which can be found on the Borough website. I confirm the he/she is in good physical condition and does not have medical issues that could be aggravated by their participation. In case of a medical emergency, I give permission for treatment by a hospital or physician. I give permission to the Red Bank Parks and Recreation staff and agents to transport him/her to recreation events, if necessary.

YMCA Camp Ralph S. Mason makes every possible effort to insure the safety of our campers and staff. YMCA Camp Mason works to conduct enjoyable, educational and safe programs that aim to further the mission and objectives of the Camp, and with the best interests of its participants in mind. However, many programs do have inherent risks and although safety procedures have been established to minimize these risks, not all risks and hazards can be eliminated due to the nature of the activities offered. I, in my legal capacity as the parent/guardian of the minor in this application, do hereby acknowledge and agree that participation in YMCA Camp Bernie activities comes with inherent risks. I have full knowledge and understanding of the inherent risks associated with camp participation, including but in no way limited to: (1) slips, trips, and falls, (2) aquatic injuries, (3) athletic injuries, and (4) illness, including exposure to and infection with viruses or bacteria.
