VOLUNTEER BEHAVIORAL HEALTH CARE SYSTEM NEW LEAF RECOVERY TREATMENT CONTRACTRecovery Treatment Contract • February 16th, 2012
Residents agree to enter into a treatment contract with New Leaf Recovery Center, Cookeville, Tennessee. Treatment shall not be denied to any client on the basis of race, color, sex, religion or national origin. If indicated, residents will be admitted to the medical detox phase for a period of one to seven days while detoxification is completed. In the case where medical detox is not indicated, clients will be placed in the social detox phase and will remain for two to five days. When medical or social detox is completed, clients will progress to the residential treatment phase. Length of treatment will be governed by progress toward treatment goals as assessed by the New Leaf staff using the American Society for Addiction Medicine Patient Placement Criteria (ASAM-PPC2R). The resident’s motivation and willingness to work the program will influence the discharge date.
