AMENDMENT NO. 1 TO REGISTRY DATA SERVICE AGREEMENT This is an amendment to the agreement dated 10Aug2015 (“Agreement”), between: Genzyme Europe B.V., Paasheuvelweg 25, 1105 BP Amsterdam, The Netherlands (hereinafter referred to as “[Sanofi Genzyme]”.and Národný ústav dětských chorob (with the original name Dětská fakultní nemocnice s poliklinikou Bratislava), Limbová 1, 833 40 Bratislava, Slovakia,represented by doc., , general director and by , medical director (hereinafter referred to as (“Institution”). hereinafter collectively referred to as the “Parties”. SCOPE The Agreement concerns Sanofi Genzyme Rare Disease Registries. This amendment to the Agreement describes(1) the inclusion of the latest Data Protection laws and regulations and (2) change in payment as per the eCRF fee schedule and enters into effect (retrospectively) as per availability of the eCRF.
