MEDICAL RELEASE: Permission is granted for any x-‐ray, examination, anesthetic, medical or surgical diagnosis or treatment, and hospital service that may be rendered under the general or specific instruction of camper’s physician or any physician the camp may call, whether such diagnosis or treatment is rendered at the office of camper’s physician, at a licensed hospital, urgent care facility, or at the camp. Parent(s), of course, will be contacted.
