This Translational Research Program Renewal Grant Agreement (the “Agreement”) is entered into as of DATE between The Leukemia and Lymphoma Society, Inc. (“LLS”) a New York not-for-profit corporation with an office located at 3 International Drive, Suite 200, Rye Brook, NY 10573 and INSTITUTION (“Sponsoring Institution”), located at ADDRESS (individually each of LLS and the Sponsoring Institution is a “Party” and collectively they are referred to as the “Parties”);
