This Rental Services Addendum (“Rental Addendum”) is hereby attached to and incorporated into the terms of that certain Multiple Service Contract (the “Agreement”) by and between Hoover Material Handling Group, Inc. d/b/a Hoover Circular Solutions, a Delaware corporation with offices located at 24285 Katy Freeway, Suite 200, Katy, TX 77494 (hereinafter “Hoover”), and [Customer Name], a [Jurisdiction of Formation] [Type of Entity] with its principal place of business located at [Office Address] (hereinafter “Customer”). This Rental Addendum is a “Service Addendum” (as defined in the Agreement) and contains terms additional to the terms and conditions of the Agreement governing Hoover’s provision of Rental Services, including the rental of all Equipment from Hoover by Customer. In the event Customer elects to utilize Hoover’s Fleet Management Services, the Equipment as used herein shall also include, but not be limited to, any associated or attached GPS and cellular equipment, such as RF
