These Repair Order Terms and Conditions (“Repair Order Terms and Conditions”) are applicable to all Component Repair Agreements, Repair Orders, and other similar documents that reference these Repair Order Terms and Conditions (collectively referred to as the “Repair Order”) entered into between Alaska Airlines, Inc., Horizon Air Industries, Inc., or any other party identified on the Repair Order (“Alaska”) and the party contracting with Alaska on the Repair Order (“Vendor” and, together with “Alaska”, the “Parties” and each a “Party”).

This Repair Order ("Order"), including the front of this Order and these terms and conditions, constitute a contract for the Service Work for the Vehicle as described on the front side hereof, between Parrotta's Auto Repair & Sales identified on the front side hereof ("Parrotta") and the customer identified on the front side hereof ("Customer"). This Order is binding upon Parrotta and Customer upon Customer's execution of the front side hereof or the commencement of the Service Work or any part thereof.

Customer's execution of the Order or acceptance of the Services described therein constitutes an express acceptance of this Agreement.
