The undersigned breeder hereby pledges and formally acknowledges breeder’s willingness and intention to comply with the Official CEB-US Hip Dysplasia Policy and to breed only dogs which meet the minimum criteria based on radiographic hip evaluations acceptable to CEB- US from the country of origin of the dog if such rating was performed outside the United States, or based on such radiographic hip evaluations performed by the Orthopedic Foundation for Animals (OFA) or the University of Pennsylvania Hip Improvement Program (PennHIP) if such rating was performed in the United States.
