This Agreement is made effective as of the 1st day of March, 2009 (the “Effective Date”) by and between Senior Whole Health LLC (hereinafter referred to as "SWH") with a principal place of business at 58 Charles Street, Cambridge, Massachusetts, 02141 and Phone DOCTORx, LLC, (hereinafter referred to as "Group”) a professional corporation of physicians, each licensed to practice medicine in the Commonwealth of Massachusetts, having an address of 200 Mill Road, Suite 350 Fairhaven, MA 02719.
