This Service Professional Agreement (the "Agreement") sets forth the terms and conditions whereby you, an independent Service Professional fully-licensed (to the extent required by applicable law) and qualified to provide the services contemplated by this Agreement (the "Service Professional"), agree to provide certain services (as described on Schedule 1) to third parties that may, from time to time, be referred to you via the web-based platform of Purify Pros, LLC., a New Jersey LLC, with a mailing address at 232 47th St., FLR 1 Union City NJ 07087 ("Purify Pros").
