Attached is a copy of the Albemarle County FY2020 Licensed Provider Agreement that enables Albemarle County to purchase your services though CSA for the upcoming fiscal year, beginning July 1, 2019 and ending June 30, 2020. All signed Purchase of Service Orders (POSOs) will be subject to this signed Agreement. Please note that before Albemarle County is able to purchase or reimburse your services the following must be submitted: 1) the completed copy of this agreement with pages 1, 14, and 15 completed/signed and 2) the required documents as indicated below and in Exhibit B.
