The Kirklees scheme recognises the importance of having a written agreement which explains the service that will be provided to meet the needs of the person who will be using or living in the Shared Lives arrangement. This agreement is known as a Shared Lives Arrangement Agreement and sets out the terms and conditions of the Shared Lives arrangement before the service begins.

Accommodation Payment the fees payable to the Shared Lives Carer for room rent as set out in Schedule 1; Additional Payment any sums in addition to the Care and Support Payment, Food and Utilities Payment and the Accommodation Payment which are due to be paid to the Shared Lives Carer under this Agreement as set out in Schedule 1 or which are payable in accordance with the Authority and/or the Shared Lives Scheme’s criteria and payment banding levels including but not limited to contributions toward petrol costs, training costs and day time activities; Approval the Shared Lives Carer being and continuing to beapproved by the named Shared Lives Scheme to be able to support individuals using Shared Lives ,

This Shared Lives Arrangement Agreement is a written agreement made in relation to each individual Shared Lives arrangement. It is an agreement between the Person who will be making use of or living in a Shared Lives arrangement, the Shared Lives carer, the scheme and anyone or organisation involved in commissioning the service. The agreement incorporates the responsibilities and expectations of all those involved in the arrangement and includes details of the plan of care for the person using the service. It must be signed by all relevant parties before the commencement of the Shared Lives arrangement.
