This SITE USER LICENSE AGREEMENT (“User License”) dated and made effective as of ___________________, 2011 (the “Effective Date”), is entered into by and between CAVITATION TECHNOLOGIES, INC., a Nevada corporation, having an office at 10019 Canoga Avenue, Chatsworth, California 91311 (“CTI”), and *****, a Maryland corporation, having an office at ***** (“*****”) (each sometimes referred to herein individually as a “Party” or collectively as the “Parties”).
