(“Agreement”) is made effective this 1st day of March, 2024 (“Effective Date”), by and between SouthernCare, Inc. d/b/a Heartland Hospice (Rockford) (“Hospice”) and Dekalb County Rehab & Nursing Center (“SNF”)(each a "Party" and collectively the "Parties").
