THIS MEMBERSHIP AGREEMENT (this “Agreement”) is entered into as of the date identified on the last page (the “Effective Date”) by and between 612 Sauna Society, a Minnesota cooperative association organized under Minnesota Statutes Chapter 308B (the “Co- op”), and the party identified on the last page (the “Applicant and upon execution of this Agreement by the Co-op, the “Member”). The Co-op and the Applicant agree as follows:

I/we acknowledge that through the inclusion of Northlight Theatre as a beneficiary in my/or estate plan, I/we are providing support for Northlight Theatre’s mission of changing perspectives and encouraging compassion for many decades to come. I/we understand that while we have currently included Northlight Theatre in our estate plan, I/we have retained the right to alter, amend or revoke this beneficiary designation in my/our estate plans at any time.
