Due to the enhanced risks currently created by COVID-19, in order to participate in the above- referenced activity (the “Activity”), the Peralta Community College District requires all Activity participants to complete and sign this Special COVID-19 Screening, Release, and Indemnification Agreement (“Agreement”).
