This Research Specimen Material Transfer Agreement (the “Agreement”) is by and between OSF Healthcare System, an Illinois not-for-profit corporation, owner and operator of Saint Francis Medical Center (“Provider”) and The Board of Trustees of the University of Illinois, on behalf of the University of Illinois College of Medicine at Peoria (“Recipient”) regarding the transfer of human specimens, with or without associated data, from Provider to Recipient, as an approved third-party end user for research purposes as further defined below. Throughout this Agreement, Provider and Recipient are collectively referred to as the “Parties.” This Agreement will become effective upon the date of the last signature affixed below.
