This State Rebate Agreement (“Agreement”) is by and between the Massachusetts Executive Office of Health and Human Services (EOHHS), the single state agency for the administration of MassHealth, the medical assistance and benefit program pursuant to Title XIX of the Social Security Act (42 U.S.C. 1396), Title XXI of the Social Security Act (42 U.S.C. 1397aa et. seq.), M.G.L. c. 118E, and other applicable laws and waivers, and the above-referenced Blood Glucose Test Strip Manufacturer, hereinafter “the Manufacturer.”
